There are various root causes: such as file was in use, permission to the file changed, network interruptions, services were stopped or restarted during processing phase, etc.
One particular confirmed root cause occurs when the source directory of a collection set has been moved or renamed. This could happen, for example, when a case has been transferred to a different appliance, but the collection set source directories have not been manually copied across. In this situation, any new sources being processed (including non-collection sources) will be processed successfully, but the glyph will not be updated. The following error will is reported in the server log during post-processing:

In this instance, to populate the glyph correctly, ensure that the missing directory is present, and then re-process the source (re-running post-processing alone is not enough to update the glyph).

The PST could already be processed but the glyph might still show some items in "Selected To Process" similar to the screenshot above.
