The issue is caused when a user is a member of multiple Domino Directory groups with the exact same name. (i.e. In a multi-domain environment there are multiple LocalDomainAdmin groups)

On the Enterprise Vault Domino Gateway Domino Server console, the following message is displayed:
Attempt to access database EV/EV_0000004.nsf By Test User/SYMC was denied
Ensure the user is only in uniquely named Domino Directory groups.
