Cause
A Dtrace of EVIndexVolumesProcessor process while performing the Index Volume Rebuild reveals the following:
{WriteReportHeaderState} The only index volume for this content source is empty
{EVRebuildSubTask} Setting RebuildEndSequenceNumber to [0]
{EVRebuildSubTask} Setting CurrentRebuildUpgradeStepID to [Switching_Completed]
The above line RebuildEndSequenceNumber equals 0, indicates a Null value in the Enterprise Vault Directory - Index Volume - HighestItemSequenceNumber field.

One or more index volumes have a status of 1- Not Set, even though there are confirmed items in the Archive. An Index Volume Rebuild succeeds, however no items are added to the index volume.
