Additionally, the Login prompt will never be presented and the 'Login' option will be grayed out in the Menu. 

This issue has been reproduced on Outlook 16.16.12 after uninstalling the EV for Mac Tool bar and installing another version of the Tool bar. It can be caused by a corrupted Plist file that was left behind from the uninstall of the Tool bar and a subsequent reinstall fails to create it. 

Work Around: 

1.  Follow the EV for Mac Toolbar Guide to completely remove/uninstall the toolbar and then reinstall.

2.  Copy Info.plist from older/working version of the EV for Mac Toolbar package and replace the one in non working package with the one copied.
