DTRACE Information:
To validate if this is the issue a DTRACE is needed, please perform the following steps on the Enterprise Vault (EV) server while re-creating the issue.
DTRACE the process EvLotusDominoPolicySyncTask and DirectoryService for no less than 5 minutes while re-creating the issue.
Open the log file created and search for either of the following the strings:

(EvLotusDominoPolicySyncTask) <45192> EV~E |Event ID: 41118 The Domino Mailbox Provisioning task 'Domino Provisioning Task for MYEV' has been aborted.
The customer migrated EVDG server from EVDG01 to EVDG02 using migration wizard. EVDG01 is no longer present in the environment but provisioning task is trying to test connection still to EVDG01.
1. Create a new Veritas support case
2. Put EV in backup mode
3. Collect full VQA so that ALL data including SQL is in the VQA and add to the new Veritas support case
4. Backup Enterprise Vault databases in SQL
5. Contact Veritas support for assistance (TSE to review internal notes)
