The 45084 event will be seen in the Veritas Enterprise Vault Event Viewer log. Also, a Dtrace on the EvNetAppCModePHSvr process during a restart of the Enterprise Vault Admin Service on the EV server will show the following:

This issue has been seen to be caused by a missing self-signed certificate on the NetApp Vserver. This will cause authentication to fail from the EV server when attempting to validate/create the FPolicy.
Ensure that a self-signed certificate is installed on the Vserver and restart the Enterprise Vault Admin Service.
