A Dtrace of the EVIndexAdminService on the EV server may show the following message.
(EVIndexAdminService) <34680> EV-L {VELOCITYINDEXINGENGINEERRORS.EN_US} Skipping report as an identical report has been processed recently. ID=LIBSQLITE_CRITICAL, Message=An error occurred while attempting to execute the SQL statement [] on the database [I:\EVIndex\IndexStorage\index7\134A544FD7AA10F4692D9AFD6B996C8F0_71\live\log.sq lt]: [database disk image is malformed]

If the above error is seen the cause is a corruption in the index volume that will not allow updates to occur.
Verify that there are no disk related issues and antivirus exclusions are in place.
A rebuild of the index volume will be required to resolve the issue. Please refer to the documentation(Additional information) for more information.
When Storage Expiry runs the items are not being removed from the index volume causing a 41022 event to occur stating items have not been deleted from indexes.
